Our story
Care One Home Health Services is a locally owned and independently operated home health agency founded by a Registered Nurse who saw the need for more compassionate, dependable, and personalized care at home.
Since opening our doors in 2005, our goal has always been simple: treat every patient like family and provide the kind of care we would want for our own loved ones. Over the years, we’ve had the privilege of caring for patients throughout our community while building lasting relationships with families, providers, and healthcare partners.
What makes Care One different is the heart behind what we do. We understand that inviting healthcare professionals into your home is personal, and we never take that trust for granted. Our team takes the time to listen, understand each patient’s needs, and create a care plan focused on safety, comfort, independence, and quality of life.

Our Leadership
Lorna Agustin, BSN, RN
President & OwnerLorna, BSN, RN, is a dedicated nurse with more than 40 years of experience in healthcare, including an extensive background in critical care and ICU nursing. Her nursing career has taken her across the world, allowing her to care for diverse patient populations and gain invaluable experience in a wide range of healthcare settings. Throughout her journey, she has remained deeply passionate about providing compassionate, high-quality patient care while mentoring and inspiring others in the profession. Driven by a vision to make a greater impact beyond the bedside, Lorna pursued her aspirations of becoming a healthcare leader and entrepreneur, helping shape a culture rooted in excellence, integrity, and patient-centered care.
AdministratorKerstin Agustin, MS, BSN, RN, CCS-HH
Kerstin, MS, BSN, RN, CCS-HH, brings a strong background in healthcare administration and operations management, combining both clinical expertise and leadership experience within the home health industry. Before becoming a Registered Nurse, she worked extensively in healthcare management, which gave her a deep understanding of the operational side of patient care. Driven by a desire to become more clinically well-rounded, she returned to school to earn her BSN and RN credentials, further strengthening her ability to lead both administratively and clinically. Passionate about advancing her knowledge and improving patient outcomes, Kerstin continued her academic and professional growth by earning her CCS-HH certification, recognizing her as a Clinical Care Specialist in the home health sector. Her approach blends compassionate patient care, regulatory knowledge, and operational excellence to support high-quality care in the home setting.
