Home Health
Home health services are designed to support a safe and smooth transition back to your home following discharge from your most recent inpatient stay. To ensure continuity of care, our team completes an Initial Assessment within 48 hours of discharge whenever possible.
Your Primary Care Provider may also refer you for home health services if you are considered homebound and have a skilled medical necessity. Through scheduled intermittent visits, our skilled clinical staff provides the care, education, and oversight needed to promote recovery—allowing you to heal safely and comfortably in your home environment.

In most cases, home health services are covered by insurance with little to no out-of-pocket cost. Prior to initiating care, our team can verify your insurance benefits and eligibility to help you better understand your coverage and any potential financial responsibility.
To begin home health services, please schedule an appointment with your Primary Care Provider (PCP) or referring physician. If you meet the eligibility criteria for home health care, your provider can submit a referral to Care One Home Health Services so services may be initiated promptly.We currently accept the following insurance plans and programs: Traditional Medicare Select Medicare Advantage PPO Plans Veteran’s Administration (VA) Community Care Program Workers' Compensation

Private Duty Nursing
Private Duty Nursing provides individualized, extended skilled nursing care in the comfort of your home. This service is ideal for clients who require ongoing clinical support beyond short-term or intermittent care, allowing patients to remain safely at home while receiving continuous, high-quality nursing oversight.
Our clinicians are carefully matched to each client based on their medical condition, care needs, personality, and level of acuity to ensure compassionate, patient-centered care and optimal clinical outcomes. Care plans are developed collaboratively with physicians, families, and caregivers to support safety, comfort, independence, and quality of life.

If your commercial insurance plan includes coverage for Private Duty Nursing services, our team will work directly with your insurance provider to obtain the necessary authorizations and coordinate payment for approved services. We handle the submission of required medical documentation and clinical information to help facilitate a smooth and timely approval process.
For clients without applicable insurance coverage, Care One Home Health Services also offers flexible private pay options to ensure continued access to high-quality skilled nursing care tailored to your needs. Our team is available to discuss available service options, scheduling, and rates to help determine the best plan of care for you and your family.Home Services
Home Services provide non-medical support for individuals who need assistance with Activities of Daily Living (ADLs) while remaining safely and comfortably in their home environment. These services are designed to promote independence, dignity, companionship, and overall quality of life.

Non-medical Home Services are typically a private pay, out-of-pocket expense; however, certain services may be eligible for coverage through commercial long-term care insurance plans. When applicable, our team is happy to assist with benefit verification and coordination of required documentation.
Care One Home Health Services offers competitive and transparent pricing across our range of home services, with flexible care options designed to meet each client’s individual needs and schedule. Available service options include daily live-in care, 24-hour care, and hourly caregiver services.
For hourly “come-and-go” caregiver services, a minimum of four (4) hours per day is required. Our team is available to discuss care needs, scheduling options, and pricing to help determine the most appropriate level of support for you or your loved one.The Care One
